Cam Phasers. Really? If it is what about the lock out

I was driving to work when my truck made a loud noise like metal hitting a fan blade. I immediately think about compression, the more you accelerate the louder it got. I shut it off the noise does not come back after restarting, the engine has power but, its running roughly, stalling when cold but runs well enough to get me home once warmed up.
I get home run the codes get a PCV valve(severed hose) and the MAF sensor that went away after changing the hose. There was no code on the misfire.:sad2:
I take it into the local garage he takes it out for a 5 minute drive tells me its cam phasers, and he begins to lecture me that they only crap out because of poor maintenance.
This may be so but I run my truck for a living. It started out on Castrol and now I use Amsoil synthetic and have for several years always scheduled maintenance and oil changes transmission flushes.
How could he diagnose this without opening the hood, using a scanner etc. He only charged me 30 bucks but really there no knock it just runs rough.
How successful is the lock out and tuner combo?
Do I have to change out my timing chain?
Is it worth it?
